Procedures and specialized services
Some services provided by a physician are not covered by Alberta Health because they are requested for employment, insurance, licensing, school or another administrative purpose. These services may require an appointment, a review of your chart or additional testing before the physician can complete the document.
Bring the original form or request whenever possible. The physician can only provide information and opinions supported by your medical history and assessment. A form may require an appointment even if you recently saw the physician, and detailed reports may also take longer to complete because the physician needs to review records, results and specialist information. Payment covers the work required to complete the service. It does not guarantee a particular medical opinion or outcome.
Ask about the expected completion time when submitting the form. Short notes may be completed quickly, while disability, insurance or legal reports may require more time. Bring documents well before their deadline whenever possible.
These services are generally not covered by AHCIP. Fees vary based on the time and complexity involved, and we will tell you the expected fee before beginning the uninsured service.
